The form should be used by life insurance companies, pension funds or credit institutions domiciled in an EU/EEA member state other than Denmark, which in the country of domicile are authorised to operate as a life insurance company, pension fund or credit institution offering pension schemes covered by Part 1 in the Danish Pension Tax Act to Danish pension holders.
