06.036 EN - Apply or reapply for dividend tax exemption card

Purpose of the form

This form is for recipients of dividend tax who meet the conditions for applying or reapplying for a dividend tax exemption card or who want to report the termination of an existing tax exemption card.

The rules on who can apply for a dividend tax exemption card are laid down in section 30(2) of the Danish Executive Order on Withholding Tax (Kildeskattebekendtgørelsen) or at Declaring and paying dividend and dividend tax.

You need to specify the recipient in the form.

Please note

Depending on which type of tax exemption card you apply for, you will be asked to attach different documents, for example current statutes, financial statements, registration certificate, declaration of domicile, etc. 

If the conditions for the tax exemption card have changed since it was issued, this must be reported in the form.
